This report must be completed and submitted to Enterprise Risk Services at (oirm@tulane.edu or fax to number above) as soon as practicable after any loss or damage to University property due to theft, vandalism, fire, windstorm, flood, hail or any other cause.
For all claims, the applicable Department will be responsible for up to $2500 of the loss or damage.
